SR Update Request-add note?

Hi,

On the webpage for an individual Service Request, there is a button for "Provide an Update" which opens up a form for you to enter a note and attach files as requested.  Near that button, is a button for "Request an Update".  Could you add a form to enter a note while requesting an update?  Currently that button just inserts a standard note "user is requesting an update" (or something like that).  I would like to be able to add an additional note to that, such as "When is this scheduled to be fixed?", or "Was xxx addressed?", etc.
