The issue with custom operations not using annotation scale is a known issue that has not been implemented yet. I was able to reproduce the problem with cells not honoring the rotation angle. I have filed a TR (D-134838) for this to be addressed.

So what else have your tried, have you deleted your GEOPAK resource files and made sure there are none of those GEOPAK resource files in your GEOPAK working directory?

I have recommend before to Bentley that the template drop graphic itself should either change color or something similar so it would be more apparent that the template is out of sync with the library. Not sure it ever went anywhere.
