We are not able to repro it, let’s start by looking at some diagnostic data. To begin with:

a. Run “MicroStation.exe -debug”. Send me the report.
b. Open any DGN file and run “SHOW CONFIGURATION”. Send me the report.

For general information, the tools in question are those within the Customize dialog, this question does not affect the ribbon or other tools.
