This problem occurs when, usually during implementation, when Work Order approval was previuolsy configured. Work order approval can be configured for various Work Types. This is done on the Approval Settings dialog's Maintnance tab. On the Maintnance tab there is a Work Orders tab with a Work Types sub-tab. The Work Types tab is used to configure work types that do or do not require approval.
When the "Work orders" checkbox is unchecked (in the "Maintenance documents that require approval at Site x" group box), the Work Orders tab with a Work Types sub-tab are hidden. However, the work types that are configured as requiring approval are not cleared, so work orders of some work types will still expect to be approved.
To fix this problem, re-activate Maintenance approval for Work Orders, and go to the Work Types tab. Ensure that there are no Work Types checked for "Approval Required". All Work Types should be unchecked. Then deactivate Maintenance Work Order approval.
