While there are several potential areas of slow down in performance, few are as drastic as fragmented indexes. In troubleshooting the performance problem, first run the APM Performance Monitor and run through the scenario you find slow.

Next, check on the results, heavy database usage with only small network traffic and few bytes returns generally indicate a database problem. Capture the data and launch an SR and include the steps you followed that illustrate the performance issue.
While Bentley APM Support can help in diagnosing problems, in the case of fragmented indexes, fragmented indexes are better avoided by having the user's Database Administrator perform regular checks and run statistics on the database to detect and rebuild indexes, etc on a proactive basis.
