Why does Copy 000000001 get created when a file is added?

This might happen because of several reasons. 

1. Copy object Type must be Master or Secondary Master
2. Media type has to be 'eB Managed' (see screenshot below)

3. Status of Copy should be set to Available

I can't think of any other that might be a cause.

Everything is configured as you have shown -and, the file container document template is selected on the object class as the designated file container template.  The Media is eB electronic (the only one in our system).

When a Tag is created, eB applies the file container and the copy, but, when a file is added, it creates a SECOND copy and adds the file to the copy.

Why?

I could not find an option to create copies - on the Tag template or the File Container Document template.

Ken, on the File Container Template you can create the copies.

Gintautas identified this as a bug earlier today. A SR should have been created for resolution.

The issue is that regardless of the definition and/or configuration of the file container or the class of the object that uses the file container, when a file is added to an object, eB creates a copy that is assigned as the default.

The 'work around' is to force users to add files directly to the copy by opening the copy and then adding the file.

Did Gintautas create the SR?

It's a bug and I have raised this internally 663759.
This issue occurs only when Repository Group in Copy object differs from ‘Default Repository Group’ (set in Director system defaults section). When you add file from web edit view or Director ‘Add file’ button you can’t specify which copy to use, so by default eB uploads file to Default repository and as default repo does not match the one from template it just creates a new one and that is not correct.
