How to enable Injuries and Damage fields

In PEM  (DOC0150) form there are grayed fields Injuries and Damage. How to enable them?
 

Steps to Accomplish

The Injuries and Damage fields are active for enquiries with category set in ENQCLMCAT parameter.  This functionality is mentioned in documentation, more details please see in screenshot below.
 

 
You can change this enquiry type by amending the ENQCLMCAT parameter in Product Options form.
