After entering the details on Work reference form, clicking on the 'Other Phase Details' tab, when 'Get from Map' is loaded few users are getting 'Failure accessing WMS theme'
This might be happening due to errors generated by WMS being cached and the errors being repeatedly sent to the user, over and over, even if the error has since been cleared. This can happen if the error was generated weeks or months before.
Clear the Map cache, so that every WMS call will refresh, and will begin caching again.
If still issue persist then you can drop map cache completely
