That way I have a file with no external references, but keeping the internal references as references.
I have done this before, but seem to struggle to do this again, any help on the dwg-export settings?

These settings work for us. We typically merge the attachments when sharing DWG format but you could change the 'Merge' settings in the attachments if you want to keep these as separate files.
The key to keep the ACAD 'model space' and 'paper space' conventions in the converted file is the 'Retained' setting for 'Masked Attachments'

As far as I can see when I open your DWS file the settings for reference attachments are the same as what we are using.
I'm not sure that I follow you when you say that you are seeing paper space with "random size"? What exactly do you mean by this?
Does the original DGN file comprise a drawing model and a sheet model into which the drawing model, with references, is referenced?
Also I am puzzled that you say you are getting two resulting files. Does this occur when you run the export only once?
When you mention you had done this before, which version are you referring to? Also, in which version are the results differing?
