TeZa from @Bentley Design Review Team has completed her investigation into Defect Report 881015 from the generic DGN/MicroStation perspective.

The green dashed line representing the terrain is missing from the sheets because the Civil Connector doesn’t process the underlying Civil Infrastructure DTMEntity element (that represents the terrain) when constructing the BIS DrawingModel to be viewed on the Sheet. This is not a general DGN/Microstation (Design Review) problem and the issue has reassigned to the Civil Connector team.

I have to keep in touch with the Civil Connector team on their progress, but so far no answer regarding the development of the solution ...

may I ask about the status of the reported defect?

If I understand right information shared by Jiri Jarolim, it was identified as Civil connector defect and assigned to relevant team. Because in civil "a terrain is everywhere" ;-), so it would be nice to know expected fix schedule (I do not recall and "defects fixed" info published for iModel Connectors).

This looks like it may be an issue with how Design Review is displaying the 2D sheet. A service request will need to be logged to Design Review then a Defect can be logged on this issue. If a small sample set of files is attached to the SR that displays this issue it would be very helpful for logging and reproducing this issue.
