What is the intended workflow of such iTwin design review? We can add annotations but how to view them in Microstation to fix found issues?

Adhoc design review - there was never an intention to loop the annotations back into the CAD tools. You can export them to CSV once the adhoc review is done.If you are using Design Review or another Powered by iTwin application, then you can use the "issue resolution" tools with the supported CAD applications to review the issues created in the iModel.

At least the exported CSV could contain coordinates as now it exports just plain text.
