Hi Bao. These are broad questions which do not really have answers that suit everybody, and I'm not aware of a flow chart that would provide answers either.
You may want to watch the QuickStart for Bentley Substation, which you can access on the LEARNserver or via CONNECT Advisor, and take the courses available in the Bentley Substation 3D Physical Site Designer Learning Path. This learning path should answer your "how to design a new substation" question.
For existing substations designed outside of Bentley Substation, you can import existing drawings into Bentley Substation using the Import Drawings tool, then use the skills learned for creating new substations to add components to the project.
Additionally, you can contact your Bentley account representative and ask about arranging for training and coaching.
