When I attach or open a dgn file containing a mix of complex shapes and grouped holes, Map Manager will create a separate inferred feature for the grouped holes called GHOLE. This feature contains elements from all layers that contains grouped holes.
Why?
This means that the other layers in Map Manager are incomplete.

I suspect the shapes composing the Grouped Hole are on different levels. Since inferred feature in map manager are recognize by level and Grouped Hole does not have any level assigned (as composite element), it will put into a default layer. I will expect more a level/layer name as Level_0. It will be good to know which version you using here.
As workaround, if it is not a restriction for your project, you could set all element composing Grouped hole on the same level. Then, Map Manager should place the grouped hole on the expected layer in map manager instead of default layer.
