No General section / Elevation row in Geopak or Inroads when importing ESRI contour into Microstation thru Bentley Map

I read the geospatial wiki:

Importing ESRI Shapefiles with Topo to their proper elevation

Following the 6 steps, however, in step 4, there is no General section / Elevation row. Please see the situation below:

Any suggestions are really appreciated.
