PLAXIS applications utilize the Bentley Licensing System. That means that since you have 12 x PLAXIS 2D licenses you can launch up to 12 PLAXIS sessions (Input/Output) in different PCs.

No, only one PLAXIS Input session (incl. calculations) can be launched per computer. Note that PLAXIS Input and PLAXIS Output count as one license being used.
Yes, one Bentley account can be used in multiple PCs, within the 12 licenses limit you own.
No, as this is limited by what is mentioned in the first bullet point.

Note that with our latest PLAXIS version we have introduced a Calculation manager to allow for running multiple 2D and 3D projects without the need to open each project and click on calculate. Here is an informative video:

Regarding virtual machines, the idea is still the same, as in the end you can only launch one PLAXIS Input session per virtual machine and you can use the same Bentley account in different virtual machines.
