I want to include a header which I had previously taken out of the project by making it not 'active' but now when I set it to 'activate' it is not showing up as a header in my project. Why isn't it displaying?

It may be that the field was moved out of the grid as well as being deactivated. Please go to System>Configuration Packs>Select the pack>Manage Model>Select the table and then "Manage Grids". If the field you need is in the window in the left, move it to the window on the right and save changes. This should bring it back if the field is marked as active.
If this doesn't help please let me know the field and I'll take a closer look.

I have moved the Header into the window on the right but it's not clear which Grid I should be editing. The header is 'Date Drilled'
