For the installation details to show in the well section of the templates you need to ensure you have entered your installation data in to both "Monitoring Installations and Instruments" table and the "Monitoring Installation Pipe Work table".
In the Monitoring Installations and Instruments table (as seen below) ensure you have the Key Field data added, this includes "Location ID", "Point Reference" and "Distance". As of build 1.26.0.6 of HBSI you will always set the Distance as the end pf pipe depth. For earlier versions you set the Distance as "0.00" for standpipes. You must also make sure you set a value for "Pipe Reference".
You can then move on to the Monitoring Installation Pipe Work table (as seen below). Here, again you need to ensure you have the Key Field data added. This includes Location ID, Pipe Reference, Depth Top, Depth base and also set the value for the common field pipe type.
The Key thing that is usually missed is the PIPE reference. You must ensure you have a consistent value across both tables. This is because its a key field for "Monitoring Installation Pipe Work table" but not for "Monitoring Installations and Instruments" table.
