Hello, I am trying to export a surface model from Civil 3D to SewerGems for a university project but when I export the .xml file to SewerGems it is misaligned to to my background layer. The red outline should be shifted to the right to enclose the coloured background layer. Any help will be much appreciated!

SewerGEMS itself does not use a coordinate system. It will rely on the X and Y values for the coordinates to place the elements. It appears this is strictly related to either the background layer or the terrain model. I would check that the scale and X-Y values are accurate, including the units. If there differences, you can use native Civil3D tools to project the files to the same location. See this link for details (though it relates to reprojecting existing elements in a model): Is it possible to reproject a model to a different coordinate system? One this to check related to the scale is to look at the corners of the background image in your screenshot and check the coordinates displayed in the lower right. If there is not much distance between between the corners of the background image, the issue may relate to the scale of the XML and the DXF background file.
I would also check the units for the Civil3D data. I have not seen this in Civil3D data, but the coordinate units are in decimal degrees, the location and scale of the model may not be accurate. Projecting the files to a coordinate system with length units, like meters, would help with the issue.
