When customizing MicroStation's user interface, it's useful to be able to appoint an icon in a tool palette. That leads to the Define Icon for Tool dialog...
The above dialog has a curious behaviour: the mouse scroll button doesn't scroll the view of icons. Instead, it remains focussed on the Option Button labelled Look for icons in.
If one uses the scroll bar on the right, then the icon view is scrolled as expected. But, using the mouse scroll button changes the selected icon source. Since most of those are empty, the consequence of using the mouse scroll button is to clear the icon view.
That behaviour is counter-intuitive, confusing and irritating. Please fix it.

I do not appear to be able to reproduce this with my install of CONNECT UPDATE 5 when editing personal.dgnlib, although in my case the dialog width is smaller only showing 5 icons across where as your image shows 6.
I am not able to resize the dialog to incorporate 6 icon width to see if this would make any difference. This being a know issue which and existing enhancement is filed see https://communities.bentley.com/products/microstation/f/273/p/131284/401480#401480
May be its hardware graphics / mouse related problem assuming your also using UPDATE 5, perhaps try changing the resolution to see if this helps.
Does the Mouse scroll work OK for you in other dialogs ?
Have you changed you mouse Button Assignments ?
