I noticed that accudraw is in polar mode and not rectangular mode. Hit the space bar to switch it and see if that works, otherwise I would rename the .upf as mentioned and see if that does not correct it.
The Cursor Icon says it is in Polar mode by the "Circle" display on the cursor. It is displaying X(Red Line) and Y(Green Line) in the Cursor icon display so you should be able to lock onto XY (Red Green) directions unless you have AccuDraw Indexing Off for the Angle. BUT the AccuDraw dialog shown denotes it is XY (Rectangular mode)?????Maybe deleting the UPF will fix????? I noted a Direction Arrow on the cursor icon also. Are you using Civil AccuDraw? Also looks like you are using M-Sta. V8i (? Version).
You may also want to check the Design File Settings for Axis Settings > Design File > Axis to see if the Start Angle is 0 and the Increment is 90.
