MicroStation Connect Edition 11 - When we try to reference a file in a MicroStation drawing it freezes and we get this message.
Unable to load/create dialog item of type 'iconCmd', id = 101 from dialog/task "MGDSHOOK"
When I update the UPF we do not have the problem anymore. so what would cause the UPF to change so it doesn't work.
UPFs have been subject to random corruption for about forever.

It's not any better with Connect. If you do much customization to your preferences, it's a good idea to make a backup
