I working in a dgn with an old PDF drawing as Raster Reference. The original PDF is 6MB in size. When I print the new dgn to PDF it blows the file size up to 54MB. Is there a way to influence the file size of a PDF before printing?

One way to reduce the size is reducing the PDF Driver Resolution. It's discussed in detail in following Wiki:

I used a combination of Driver Resolution (Dots/Unit) and Raster Quality Factor (0-100%) in the Printer Driver Configuration file to achieve a much smaller file size without compromising the output quality too much.

That's the answer. Keep in mind there is no perfect setting, you may need more than 1 pltcfg for different situations. You should be able to use just one for your most typical work.
