If file contains large mesh element I'm no longer able to force Accusnap using Shift+Ctrl shortcut. This makes hard to use view rotation tools as the anchor can't be fixed to desired point.
Seems there is some file size limit after which accusnap is disabled?
Check your AccuSnap settings. Verify the Meshes checkbox is toggled on.

Everything is enabled. It also doesn't allow to snap to any other element type. Very strange.
File is retouch DGN exported from ContextCapture and is 30MB in size.
Does tentative snap work? Also, what is considered a "large mesh" (approx # of vertices)...

Tentative work but very slow. Not sure have to check how many vertices but most likely maximum supported.
It sounds like you just have a very large mesh that AccuSnap is trying to slog through while constantly having to start over every time you move the cursor. Probably if you take your hand off the mouse AccuSnap will show the snap point after taking roughly the same amount of time it took to tentative snap.
The best thing to do when working in this model might be to disable AccuSnap for meshes so that it doesn't affect snapping to other elements with ranges that overlap the mesh and just use tentative snap on the mesh when necessary. If splitting the mesh is an option that will make a huge difference as well.
