it seems you are right:: the keyboard shortcuts definitions are loaded only when MicroStation loads workspace. It's not exactly "to restart microStation" as you wrote, because the file is reloaded also when a file is closed (but not MicroStation) and a new file (plus workspace/workset optionally) is selected.

It'll be much more efficient for me to edit the xml in text editor than using the CE GUI to migrate the old shortcut.txt and adjust to new CE keyins. We also have different sets of shortcuts in V8i for different usage, and switching between versions of shortcut.txt was/is easy as in V8i.

I agree to use an existing xml definition and to modify it directly can be an efficient way. Even though to edit xml is generally bad idea and never recommended, the syntax is very simple.
Moreover, it looks like that xsd file is delivered with MicroStation. It's not mentioned in created xml files and looks like it's defined for a bit different purpose, but it can be used as a reference, because supported ScanCodes are mentioned there.
The migration is time limited task, not done regularly, so to close / open design file to reload the shortcuts is acceptable workaround in my opinion.

In CE, it's "per user", because the definition is created as a part of user preferences. But using configuration variable, different shortcut configurations can be defined for individual worksets or workflows.
I think there is no way how to reload the definition in runtime, and at least to close / open the file is required. But it's not technical limitation, because when edited from dialog, the changes are available immediately when Apply button is clicked. Only such key-in is missing.
You should add this request to Ideas section, so other users can vote for it and to increase a priority.
