I'm in a drawing that has the "Line Style Scale" set to "Global Line Style scale" with a value of 1.000
And after typing "active linestylescale" into the Key In, it says my Active Linestyle Scale is 1.000
But when I offset a line using a custom line style, that has the scale set to 1 in the Line Style Parameter, or draw a line, the new line has a a line style scale of 1.5.
Annotation scale is turned off but is set to full size.
Is there another setting I need to check or is there another solution to get my lines to draw at the right scale?

You can check a couple of other places.
The Custom Linestyle dialog has a place to enter a linestyle scale. While this may be the Active Linestyle Scale, it never hurts to look.
Your level may have a linestyle scale defined (in addition to the global setting). You can find that in the "Styles" tab in the Level Properties dialog.

Thanks Mary! Once I opened that window the "Scale Factor" box what checked with a value of 1.5. Once I unchecked the box it worked.

Our linestyles are set up so that nobody should have to change the scale factor that way, but some people still do.
We ran across the line style scale being applied to individual line elements directly. It was a sub-consultant doing utility work. We started a new project in CONNECT and they were using an old v8i line style library. The old line styles were created in a file that was set at 1" = 1'. Our CONNECT files were set at 1" = 1" so they were compensating for that difference. It took us a while to figure out why the line styles were not correct and where the sub-consultant applied the scale factor.
