I think that you have run into a limitation of Item Types. We include multiple item tagging with our TagWorks family, such as AreaAnnotator...
It works with either a fence or a selection set. When tagging multiple objects, AreaAnnotator increments the object's ID automatically.

It works fine for me in MicroStation CE U16. Can you share a test case? Maybe there is an element or Item Type setting that breaks the attachment.
Alternatively, you can define Element Type with only Item Type defined (remove all attributes and other settings) and to apply it to the selected elements with no template association.

How about a fence? Does MicroStation's tool work when asked to attach an Item instance to each element in a fence?
Indeed it works for other files. But now opened original file and it works there also. Seems found how to repeat this behaviour. The Item types are defined in attached dgnlib so when first used with selection set it only attaches to one element as the Item is not yet saved in file. Will file a SR.
Elements are lines. So create a new empty file and draw some elements and then try to attach Items from dgnlib(will show as gray icon without green checkmark if not yet used). After first element is tagged it shows with green checkmark and can be applied to selection.
Another issue if selection set already contains one element with attached item the list is empty so need to find and deselect the element which now has Item.
