What is the exact version no. (e.g. v10.x.y.z) of MicroStation that you are using? Use key-in VERSION to see the version no. in the MicroStation Message Center, or Help→About MicroStation in the backstage.

It's impossible to tell what's going on from a screenshot.  Please attach a DGN example to your post.  Use the Insert=>Insert video/image/file button.

If you can't post an example DGN, then we can't help you.  Log a service request (SR) with Bentley Systems and submit your DGN file as confidential evidence.
