Can anyone tell my why the size of an arrowhead on dimensions needs to be defined and why it just doesn't use the cell as defined? This has been the case since we started using dimensioning way back when...

A terminators size (eg the arrowhead) is - since way back then - always related to the dimensions textsize. Regardless if it is a particular cell or "default".
If you use different units and scales this makes sure that the terminator always somewhat fits to the size of the dimension. If that would not be related it may become easily way too large or too small.
The fixed (arrowhead) size would also have a major effect on the Annotation Scale.
