Some default linestyle print as hole lines.
Example the linestyle 1.

I'm not sure what you mean by "hole line". Could you describe that further or post an example?
The linestyles 1-7 are defined in the .plt or .pltcfg file so can be printed to look like pretty much any dash-dot configuration regardless of how they display on the screen. Check on that.
The pen table can also do some resymbolization, so check to see if that is a factor or just detach it and see if it helps, thus limiting where the problem lies.
