I have defined my paper sizes in printer.pltcfg (ANSI A and ANSI B) and set the default to ANSI B.
When I try to print, the print dialog only has the first paper size (ANSI A) in the list as an available choice. If I move ANSI B to the top of the "Define paper sizes" list then it will be the only option in the print dialog. Why is the print dialog only displaying the first paper size of the list?

This is your windows system printer so I think you also need to make sure that windows itself has printer with those paper sizes available too... setting up windows printer over network often needs IT permission to make them save.

Windows definitely has both paper sizes available. I can print just fine; both sizes just don't show up in the dialog at the same time.
