it seems that several (personal) settings have changed. the most important on is the plot date stamp.
I've followed all the directions I can find, with no luck.

Lorys,
Thank you for helping out. No, its on a proper layer.
Due to our setup, I choose an inside border line for the plot. This has worked for 17 years ... and now, it's not.
If I choose the outside border, the plot stamp does show tho.
Hi Lisa,
Can you attach a sample DGN and a pentable so we can investigate?
I found the problem. The stamp setting for the plotter was turned off. We assume that when the plotter driver updated, it disabled some settings.
Thank you All for your time!
No worries, glad you found the solution

so your not using text substitution thru pentable and in the actual title block, personally IMHO I find its a better method and can add more options and works even with sheet models

I am using the text substitution thru the pentable as recommended.
This was a case of the wide format plot device settings being disabled.
interesting ......can you screen shot the settings? as this has not happened to us yet ... as I thought the pentable wasnt effected by the printer device settings unless it was the printable area...I just want to make sure it wont happen here with wide format printer too... so I wanted to see where it could come from in settings just incase IT or others update stuff without my knowledge.. thanks and glad you figured it out and shared it here.. could be a real easy trap and hair puller to figure out...
