There are several actions that cause this. For example:
Using a database from a previous release,
The disk is full, or
Using a database for a run that did not end "gracefully".
When I get this error I usually delete the database and start the run fresh. I usually do not try to figure out what generated this error.
It is usually faster to just begin fresh.
If you could tell us more about what you were doing when this error occurred we may be able to help.

Issue reported in MOSES 11.XX on Windows 10
This error is often related to a compatibility issue between MOSES and CISCO AMP. This is most likely due to the file extensions in the MOSES database folder. The database folder contains a number of binary database files that are used to store information for the MOSES Fortran solver. All of these files contain the extension *.000, but depending on the analysis the extension of these files could be indexed, e.g. *.001, *.002 etc. This is the case particularly in time domain analyses.
We recommend excluding certain files and folders from the AMP software and more specifically all *.dba folders found in your working directory.
