Why and how to avoid?

Solution:

In one particular report case, the computer used Nvidia graphics card where the driver had just been updated from Nvidia drivers v31.0.15.1766 to use the new driver; Nvidia drivers v31.0.15.2727 or newerOperating system and graphics card model do not seem to be factors.

The solution to this reported case was to role back to the previous graphics card drivers.
