Does anyone know if OPM comes delivered with a pump suction diffuser? I can't find it anywhere.
If it's not included, what's the best way to add a new component? Create custom component?

In OpenPlant Modeler, a derived class can be created and named as desired but graphics for pump be of OpenPlant Modeler.
To create a pump with desired graphics, you can create custom component. Please refer below wiki article.

This is not a pump. It's a pump suction diffuser. The graphics would not match any existing components in OPM. I'd need to create an entirely new component type. Is that possible using 'create new custom component'?
Yes Mike you can create it.
It would be great if you could create SR and supply the custom graphics data.

My company occasionally has to create 3D components that aren't in the catalog. It's been a while for me but you should be able to create what you need with a custom component. First you draw it in 3D. Then select the items and click the Create Custom Component button. The important part is selecting what kind of item you want to base it on. In the simple example I created below, I made a 90 degree angled item and based it on a generic Angle Valve. This gives me a Main port and a Run port and I can put those in wherever I need. Make sure you put in a class or the ends won't work correctly with the mating flanges. It might take a few tries to create it and we often have to update the description of the item after it's inserted in the model because I think it would call this an Angle Valve because of what I chose. Once you get the hang of creating components, you can make new one in just a few minutes.
