Is it possible to determine in VBA which file(s) are currently selected in the VBA dialog box or is the only way I can manipulate attachments is to loop through them or name a specific file name?

In general, VBA and MicroStation dialogs live on different continents. It's hard to interrogate MicroStation's dialogs even using MDL.
The problem is that the data displayed in the References dialog is not, in programming terms, the data in the Attachments collection. They are both copies of something else. Asking one to talk to the other would be like photocopying a paper drawing to obtain two copies, then drawing something on one copy and expecting it to appear on the other copy.
