COMMENT STRIPPED WHEN DOWNLOADED FROM ANNOTATED PDF

When 'Download with Summary' an annotated document - only part of the Subject or Description or responses -not the all the annotation comments and notes show (please see screenshots above) 

Has anyone else had the same problem? and if so how can we fix this to download all of the annotation comments and notes (As per first slide) ?
