Change the Transmittal/Submittal form

Question

I'm being ask to change the transmittal/submittal form use for review 
 Is this possible? 
 
 Change being ask is add a field, remove another one or event have two form based on the type of review 
 Also some of the fields are gray out and we would like to have them available at any point in time of the process. I understand that when in unresolve-Draft the reviewer can change them, but when back to the issuer we would like to have them update one of the field.

Also is there a place where we can see the intent use of those field in a real life scenario? 
 Example: 
 &raquo; If the field Response is set to Agree, but the document is mark as reject in the review, it kind of give a conflict information. 
 &raquo; In the field Level &laquo;... 1 - Comments and/or general remark. Refers normally to editorial mistakes ...&raquo; not sure where this phrase would be used and in which context?

You can modify the default submittal issue form or create a new form in Forms service. 
 
 All the approved forms under Submittal Issue section will be visible when performing PDF reviews in Deliverables Management service. 
 
 Submittal Issue attributes modification is only possible when issue is in Unresolved-draft state. Once it is sent with initial response for the document, the issue attribute values cannot be modified. You can only comment on it, add attachments and change it state.
