Polyplate with bottom face at different elevation

Bottom face four points are at different elevation and top face four points at same elevation

The all 4 vertical faces should be vertical not inclined'

You can achieve that by first modelling a normal polyplate / plate and then using the Modify tool to apply a taper or any cut, as per your requirement.
