How can i create 3D rail with sleeper in Open Rail CE 2021 release 1 (10.10.01.03).
Inner rail distance 1.003m with rail typ 46E1.

Nothing should be really different than for other gauge.
You must create your rail profile (template) with 0,0 on the the top, center of the rail. Rail must be vertical. We are considering adding in the future the capability to place rails with true angle.
Set the gauge and rail center to center distance in the design file settings.
Use the create rail command then the create sleepers command
Well i have change the settings but it don't work again the distance is 1.50m.
I just tested again in 2020 R1 and it looks to work as expected.
Did you do the same?

I have the same problem as S.Hasen, there are geometries that cause problems. I just made an inquiry.
In my case the "V1-R01(2)" and "V2-R01" of the attached alg file. In the dgn I have created a new alignment ("V1-R01(3)") in plan and grade from the originals of the alg and the SI track width is that of my settings (1,000 m). The problem now is how to copy the superelevation from the alg's original alignment ("V1-R01(2)") to the new "V1-R01(3)".

When you import from ALG, always check the parameters for the cant that is imported in OpenRail.
Projected cant is not exactly a copy. If you remove the source cant (the one you projected) then it also removes the projected cant. Not sure it is something you want. We don't have any tool to do a copy
