To determine plate thickness in tie down plate model, which values I should use to calculate actual plate stress and compare with allowable stress? Please see my attached pdf.

For option 1 mentioned in your PDF file, you need the stress denoted as MAX ABSOLUTE which is described here

For checking the out-of-plane effects, you need to look at the terms
axial + bending :
