1. Is there any reason that max. rebar considered for two pile cap is 6mm?

Seems like an issue with the 2 pile configuration of Case 2.
I will get back to you with the reason for this issue. For the time being, you may change the configuration to case 1 or any other configuration, it should work with no issues.
