What about if files are referenced? If they line up, try turning off True Scale and see if a ratio appears on one of the two scale factors. That would indicate some type of working units issue. We also sometimes have labeled grid coordinates in two files using grid lock with a 50' grid unit and compared how not only do the files align but how the grid coordinates align.

In the Project Options, bump the Scale (Precision tab) to its maximum - 0.123456789012.
Then, look at the Project Defaults again. Mine was set to only 4 decimal places, so either 0.99996 or 1.00004 look like 1.0000.
I use a calculator to generate this scale factor - 1.000040101019792
When I enter that as my Default Grid Factor, the ALG reports using Review Horizontal Alignment the same data as ERROR_GEO.TXT.
Also notice that file (ERROR_GEO.TXT) reports an Input Factor 1.000. But your new output should report something similar to my number.
Set your Default Grid Factor to 1.0 exactly. Press Apply, Close the dialog box.
Close and reopen the Review Horizontal Alignment dialog box. You should see two things. There is no Input Factor listed anymore (If its 1, why does it need to report it?) The alignment information now matches the file NO_ERROR_GEO.TXT.
There can be multiple Project Defaults - make sure all are set to 1.0, unless there is a known reason for it to be otherwise. This kind of setting might come from a Survey Directive. So you may need to check first.
