In my user preferences I have Automatically Save the Design turned off. ORD saves the design anyway. This is a huge issue if you open an older design you get the error message that it cannot open an older model and exits. It saves when exiting, now you can't open design in the older version either.

I tested this on my machine, and you are correct. When Auto-save Design Changes located at File > Settings > User > Preferences > Operation is unchecked, it will still save the dgn when exiting and selecting No when prompted to save. I filed this as Defect 708344: Unchecked Auto-save Design Changes, when exiting and prompted to save, ORD will save design when selecting NO. It is in development's hands now, and they will work through addressing this defect.
As far as your other point... In ORD 10.00.00.120, when a dgn from SS4 is opened, it will only open in Read-Only mode. So there is no ability to make changes and save the dgn since it is read only. Development is working to have the capability to bring SS4 dgns forward in a future release. So as of now, there should be no problem with opening a SS4 dgn as read only in ORD, and then opening it in SS4. I tested this on my machine just to make sure.
For now, I suggest making a copy of a dgn that you are wanting to make changes in. That way if you want to revert back to an older version you can do so without selecting not the save.

Was this defect ever addressed? I have a couple of users that is having this exact problem with ORD version 10.10.21.004.
