I am creating plan & profile sheets in GEOPAK SS4. I am not using a motif, just references in the sheet clip file. The issue I am having is that it is placing plan view references not only in the plan view, but also the profile view and tabular port. It's also placing the profiles references in plan view. Any ideas what is causing this?
If you are not using a motif file, the program will include any references attached to the design file you place your clipping borders in. Make sure you only have the references you want in that file and do not use reference nesting.
You should create a new dgn with the same seed as the plan, and in there, attach the plan and the profile dgn and any other references you want. Then, place your clip borders in that new file.

Yes, that's exactly what I am doing and have been doing for many years. I've never seen this happen before.
have you tried deleting your resource files?
Delete everything under the 8.11 folder
C:\Users\ yourusername \AppData\Local\Bentley\MicroStation\8.11
Is there another path to resource files that might become corrupted? This is happening to me again.
