If you have used it previously, it gets copied from DGNLIB to the file - and is probably still there so that gets used. I have created an Annotation Group and multiple Annotation Definitions. If I change an Annotation Definition using manage Annotation Group, everything seems fine in that current dgnlib file. I create a new dgn to load the dgnlib. I reference my alignment which contains an alignment, existing terrain profile and design profile. The reference does not contain any sheet models. In my new dgn, when I create the profile sheet model it is pulling the annotation group settings from the reference. In order to load my new settings, I have to create an entirely new file, recreate a new alignment, existing terrain profile, and design profile. Then I can create the sheet models from Named Boundary and it will use my revised Annotation Group settings.
