How can i create a plan and profile sheets in which the profile has bands with information like given in attached image.I am only able to get only profile in all templates i have

First things first: what version of the software are you using? Secondly, are you using a local WorkSpace or a DOT/ Agency WorkSpace?

In Profile Annotation Group, "Framed Profile" should able to give you the frame under Profile.
About the aerial or background image, you need to create separate DGN with attached raster files.
You may need to clip reference for the raster DGN in the Drawing Model.
Aerial Image should be displayed in Sheet Model.
I believe that Jimmy has the information you require here, Atif. There are a couple of profile annotation groups delivered with the software, and the framed annotation group- as Jimmy has shown - is the one that I believe you want to assign.
