I've been trying to annotate some curb ramps, and as you can see in the picture below, we show the slope and an arrow showing where the slope is going. This is an old image, but with civil labeler I was able to produce a note that tells you positive or negative slope, still, the arrows are something we need to show. Does anyone have an idea how to produce this arrow automatically with Civil Labeler or is this something we need to to by hand? Where this image comes from is an old MicroStation file from another association.

Your approach is more editable and it works well with the program, manual even if it takes longer is a safer way to annotate. A question I had for you is what do the rest of your settings in your dimension style look like? I can't quite find where to set the leader to "underline" but I was able to match the settings that you sent here.

Forgive me - I use these settings with the Place Note tool, not an actual dimension. I believe if you Place Note, you will get a note with an arrowhead and text above the line.
If you prefer to use your dimensioning tools, you could set up a separate dimension style with these settings:
This will give you a dimension with no extension lines, and an arrowhead on only one end (the "start" point).
You can also use your regular arrowhead dimension style, and alter the Tool Settings on placement like
