Sounds like maybe your proposed dtm was not emptied prior generating your surface again from the Roadway designer. If it wasn't emptied, it could leave components from previous iterations in the dtm which could explain the multiple components showing…

I'm not sure what you are expecting the Offset Tolerance to do for you but the point that you feel should be annotated is closer than 0.1m as best I can tell from the figures. Offset Tolerance is meant to remove that point from being annotated.

The problems that I noted should only be in V8i. If you are having problems then you may want to contact support with your dataset.
Also, it may not fix your collision free text problem but you should upgrade to version 08.09.03…
